Dr. Scott Graham graduated from the University of Missouri School of Medicine in Kansas City and performed his family practice residency at Southern Illinois University affiliated hospitals in Springfield, IL.
He has more than 35 years of experience, is a board-certified member of the American Board of Family Practice, and participates in professional memberships with the American Academy of Family Physicians, the Kentucky Academy of Family Physicians, and the Southern Medical Association.
Dr. Graham sees patients at Caldwell Medical Associates Family Practice Clinic in Fredonia, where he and his expert team are dedicated to providing specialized family healthcare for the whole family, from infants to senior care.
He’s called Kentucky his home for many years, and loves living here and serving the patients of our community. n his spare time, Dr. Graham enjoys traveling, outdoor activities, and spending time with his family.

Sunny Neal, MSN, APRN, FNP-C is a primary care provider serving Lyon, Caldwell and surrounding communities. After completing her Bachelor of Science in Nursing in 2010 from Murray State University she went on to complete her medical degree at Indiana Wesleyan University with a Master of Science in Nursing in 2019. Sunny is an active member of the American Association of Nurse Practitioners and the Kentucky Association of Nurse Practitioners and Nurse Midwives.
Sunny specializes in caring for patients of all ages and provides preventative care including routine checkups, immunizations, health risk assessments and more. In her spare time, she loves working on DIY projects, crafting, and traveling to Disney World with her husband and two sons.
