At Caldwell Medical Center, your privacy and the security of health information is a top priority. You are required to present picture identification and sign a release of information to obtain a copy of your medical records. No one else can sign a release for your records except you, including your spouse. In the case of divorce, proof of custody must be presented to obtain records on minor children. Legal documentation must be presented to obtain records on a deceased patient.
