The Admission and Registration Department is located just off of the lobby near the main entrance to the hospital. After hours these services are available through the Emergency Department registration. During the admission/registration process you will be asked for personal and insurance information. You will be asked to sign a General Condition of Admission, Consent Form, Assignment of Benefits and a Financial Agreement. If you are being admitted, an ID bracelet will be placed on your wrist. This ID bracelet is for your safety, so we ask that you not remove it.
Federal regulations require hospital personnel to ask you about an Advance Directive. An Advance Directive includes the presence of a living will or durable power of attorney. Information on Advance Directives is offered to every patient upon admission. In addition, you will be given a patient privacy notice that will explain your rights under the Health Insurance Portability and Accountability Act (HIPAA).
Outpatient Registration is also located just off the main entrance of the hospital. The clerk at the information desk will direct you to one of the registration clerks and then will notify the specific department that you are here for your procedure. The procedure you are having may require a pre-certification of prior authorization from your insurance carrier. This pre-certification is generally obtained by the referring physician’s office. If this pre-certification or prior authorization is not obtained prior to starting the procedure your insurance carrier may not pay for the service.
The scheduling of procedures is coordinated with the physician’s office and our Central Scheduler.
