If you no longer meet the requirements to continue your stay at a hospital facility but are not ready to be discharged to home, home health, or the nursing home, transitional care might be for you. This program allows you to keep a high level of dedicated care with the comfort and convenience of being close to home and more accessible to your support network.
Our transitional care center offers 24/7 nursing care and an assigned physician as well as administration of medications and meals. Radiologic and medical lab testing is available to qualifying patients as needed. We can also provide access to rehabilitation and therapy services.
